Job Summary
Provide exceptional care, disease management, and health education to patients while supporting goal setting and creating personalized action plans. Conduct regular outreach to support engagement and elicit behavior change, coordinating with clinical team members to ensure an exceptional patient experience. Develop and maintain professional relationships with patients and team members, creating and distributing health education materials as necessary. Work with a cross-functional product team to develop and improve the in-app patient experience. This role requires a bilingual (Spanish) Licensed Practical Nurse with at least two years of nursing or patient-facing experience. Full-time remote position with competitive compensation, including health benefits, 200 hours of PTO, and an annual CEU stipend.
Required Qualifications
- Compact state license
- Bilingual (Spanish speaking, reading, and writing)
- 2+ year of experience as Licensed Practical Nurse or any Nursing license
- 2+ years of experience in patient-facing or customer-facing roles
- Excellent customer relation skills
- Written and verbal communication skills
- Knowledge of medical terminology
- Proficiency of general medical office procedures
- Familiarity with digital applications like Slack, Coda, Google Workspace, etc.
- Strong analytical and proactive problem solving skills
- Self-motivated, results-oriented and strategic thinker
- Personal passion for health and wellness topics
- Must be authorized to work in the United States
