BH Clinician (Case Manager)-REMOTE

BH Clinician (Case Manager) responsible for behavioral health care management across the continuum, assessing clinical needs against guidelines, coordinating plan implementation, monitoring services inside and outside the network, advocating for members, documenting per standards, and collaborating with multidisciplinary teams via telephonic or in-person communication. Requires at least two years of behavioral health clinical experience and an active NJ license (LCSW/LMFT/LPC or RN). Travel is required; remote work is supported for this role. The role emphasizes coordination of care, utilization management principles, and ensuring medically appropriate levels of care throughout the patient journey.
