Job Summary
Behavioral Health Case Manager (FEP Population Health) supports members with behavioral health needs via telephonic engagement, ensuring coordinated, high-quality care. Responsibilities include utilization management for complex cases, acting as liaison between members, providers, and care teams, overseeing complex cases to implement interventions, and performing comprehensive case management activities (assessment, planning, implementation, coordination, monitoring, outcomes, evaluation). Role emphasizes empathetic communication, proactive problem-solving, and adherence to workflows/policies; may require occasional late-shift rotation for RN case managers (9 am–6 pm CST / 10 am–7 pm EST). Fully remote, remote-first, with Chattanooga, TN as potential relocation location. AI-enabled tools usage expected within established workflows.
Required Qualifications
- Current, active unrestricted Tennessee license in Nursing (RN) or behavioral health field (Master's level or above) (Ph.D., LCSW/LMSW, LLP, MHC, LPC, etc.) required
- RN may hold a license in the state of residence if the state participates in the Nurse Licensure Compact
- 3 years - Clinical behavioral health / substance use disorder experience required
- 1 year - Familiar with community care resources and levels of behavioral health care available
- Experience with Motivational Interviewing Techniques and Adult Learning Styles
- Certified Case Manager (CCM) credential currently held or must obtain within 2 years of hire
- Proficient in Microsoft Office (Outlook, Word, Excel, PowerPoint)
- Strong communication, interpersonal, organizational and analytical skills
- Ability to engage diverse populations and prioritize member needs
- Driver safety program eligibility if operating company/ personal vehicle for business
