Job Summary
Behavioral Health Care Coordinator within an integrated behavioral health care model providing care coordination, case management, and resource support to a diverse patient population with complex behavioral health and social needs. Responsibilities include facilitating direct patient care, triaging patient needs, coordinating referrals to higher levels of care, documenting services in the EMR (eCW), developing and maintaining resource directories, and collaborating with a multidisciplinary team of behavioral health clinicians and medical providers. Role involves remote work with occasional in-person meetings, travel to assigned clinics in Virginia, community networking, and participation in trainings. Requires strong customer service, crisis management, and coordination across healthcare and social services, with preference for Spanish-speaking candidates.
Required Qualifications
- Bachelor or Master Degree in social work or a human service field
- Experience serving patients in a public behavioral health or medical setting
- Mental Health first aid certification (can be obtained after employment)
- Excellent customer service skills
- Knowledge of behavioral health crisis response and triage
- Ability to document services timely in an electronic medical record
- Willingness to travel to assigned clinics and participate in in-person meetings
- Spanish-speaking preferred but not required
