Job Summary

Develop and implement bereavement care plans with the interdisciplinary team. Conduct post-death bereavement assessments and provide grief counseling to family members and significant others. Facilitate support groups, coordinate follow-ups, and provide referrals to community resources. Assist with memorial services and bereavement education for staff and the community while maintaining accurate documentation and collecting program data. This role supports the physical, emotional, and spiritual well-being of families following a loss at Good Shepherd Hospice, a member of Catholic Health.

Required Qualifications

  • Master's degree in Social Work, Counseling, Psychology, Mental Health Nursing, or related field
  • Minimum 2 years of clinical experience, preferably in hospice or bereavement care
  • Current New York State professional license
  • Knowledge of grief counseling and hospice care principles
