Bereavement Counselor

Provide grief counseling and support to bereaved children, adults, families, and persons with serious illness, delivering continuity of care between direct-care and bereavement components. Conduct bereavement and ongoing assessments based on risk indicators, develop and maintain plans of care in consultation with the interdisciplinary team, and facilitate support groups and community outreach programs. Deliver emotional, psychosocial, and spiritual support before and after patient death, including crisis intervention, follow-up calls, and referrals to community resources. Research, organize, and coordinate special grief services as assigned, while following up on reported complaints or matters of concern. Requires a Master's degree in Social Work, Counseling, or Psychology, one to three years of relevant experience, and Colorado licensure. Full-time, Monday-Friday schedule in Ft. Collins, CO.
