Job Summary
Conduct clinical assessments, interventions, and education for bereaved families of hospice, home health, and palliative care patients to enhance coping and functioning. Facilitate support groups, grief workshops, memorial services, and debriefing sessions while developing personalized care plans and managing high-risk client referrals. Complete clinical documentation, follow-up contacts, and secondary reports in a timely manner, adhering to strict confidentiality and ethical standards. Monitor program effectiveness, create resource banks for community clients, and participate in ongoing professional training. Work flexible hours including evenings and weekends as needed.
Required Qualifications
- Master's degree in Social Work (MSW) or Mental Health Counseling from an accredited school/university
- Minimum of one year of post-MSW/MHC experience in a hospital, home health, or hospice or equivalent setting
- Strong ethical standards
- Appropriate professional demeanor
- Experience in individual, family, and group treatment modalities
- Theoretical knowledge of human development, end-of-life care, family systems, and grief and loss
- Ability to remain calm and professional and troubleshoot/handle client/family or staff crisis situations
- Reliable transportation to perform tasks and responsibilities in a timely and appropriate fashion
- Proof of automobile insurance
- Valid Florida driver's license
Desired Qualifications
- Bachelor's degree in social work while pursuing a Master's degree
