Job Summary
Facilitate comprehensive linkage and follow-up between clients and the Cuyahoga County Department of Human Services, Social Security, the Marketplace, and other benefit providers. Research clients' current Medicaid status, coordinate application processes, and assist with completing forms, referrals, and documentation. Verify insurance coverage, explain eligibility, and act as an authorized representative for Medicaid and Food Assistance on behalf of clients. Conduct outreach and education to raise awareness about coverage options, provide referrals to consumer assistance programs, and monitor sliding fee discount programs. Maintain accurate EMR records, communicate entitlement status to case managers, and attend continuing education trainings.
Required Qualifications
- HS diploma or equivalent
- related experience in working with entitlements
- Demonstrate and maintain expertise in eligibility and enrollment rules and procedures
- training in the Marketplace
- passed the test for Certified Application Counselors
- Know and understand the range of qualified health plan options and insurance affordability programs
- valid Ohio driver's license
- sensitivity in relating to persons of varying backgrounds and to those who have experienced psychiatric problems
- Ability to maintain appropriate and required confidentiality ensuring the integrity of sensitive information (written and/or verbal)
- Ability to work on a variety of projects concurrently
- Ability to handle a diversified workload
