Job Summary

Assess each person receiving services for their hopes, dreams, and desires to provide supports that facilitate achievement and community participation. Ensure rights are protected by assisting individuals in developing advocacy skills and utilizing personal advocates. Provide support to meet emotional, physical, and social needs while facilitating person-centered relationships that promote self-determination. Implement the individual's plan fully, document goal completion, actions toward outcomes, incident reports, timesheets, and daily progress notes. Assist with day-to-day home operations including housekeeping, meal preparation, laundry, shopping, and vehicle maintenance. Drive and travel as required to arrange transportation, conduct safety drills, inspect equipment, and practice emergency procedures. Facilitate access to community resources such as recreational activities, housing, food, clothing, school programs, and vocational opportunities. Assist new and current staff with orientation, mentoring, and training. Attend meetings and training as required while maintaining certification in all agency, state, and federal training requirements.

Required Qualifications

  • High School Diploma
  • Valid Drivers License - USA
  • Experience working with children or adults (dependent upon population served) with a mental health diagnosis, an intellectual/developmental disability, or substance use disorder
