Job Summary
The Behavioral Health Therapist collaborates with the multidisciplinary care team to support the psychosocial and emotional needs of patients. Responsible for conducting functional assessments, developing individualized plans of care, leading individual, group, and/or family therapy, and facilitating discharge planning to ensure continuity of care. Plays a key role in patient and family engagement, crisis intervention, and care coordination across the continuum. Responsibilities include psychosocial assessments, treatment planning, documentation, participation in staff development, responding to crises, and maintaining compliance with policies and standards.
Required Qualifications
- Master's Degree in Social Work, Counseling, Psychology, or related behavioral health field
- LMSW - Licensed Medical Social Worker - State Licensure required or LPC - Licensed Professional Counselor required
- BCLS - Basic Life Support required
- De-escalation training certification within 30 days of hire required
