Job Summary
Behavioral Health Therapist PRN collaborates with the multidisciplinary care team to conduct psychosocial assessments, develop treatment plans, lead therapeutic interventions (individual, group, and/or family therapy), and facilitate discharge planning to ensure continuity of care. The role supports crisis intervention, care coordination across the continuum, and ensures accurate documentation of clinical interventions and communications in compliance with regulations. Responsibilities include engaging patients and families, participating in staff development, and contributing to resource navigation for post-hospital care.
Required Qualifications
- Master's Degree in Social Work, Counseling, Psychology, or related behavioral health field required
- LMSW - Licensed Medical Social Worker - State Licensure required or LPC - Licensed Professional Counselor required
- BCLS - Basic Life Support required
- De-escalation training certification within 30 days of hire required
- ACLS - Advanced Cardiac Life Support preferred
