Job Summary
Conduct comprehensive psychosocial assessments and develop individualized treatment plans for patients and families. Provide short-term therapeutic counseling and crisis intervention while applying protective service mandates for adults and children. Collaborate with physicians, nurse practitioners, and care managers to ensure integrated care and coordinate services across levels of care. Maintain accurate clinical documentation in the EHR and utilize de-escalation techniques as needed. Participate in team-based care meetings and continuous improvement initiatives to support smooth transitions. Join an interdisciplinary team at Jane Pauley Community Health Center, a Federally Qualified Health Center serving underserved populations across Indiana.
Required Qualifications
- Master's Degree in Social Work, Counseling, or Marriage & Family Therapy
- Relevant experience in healthcare, mental health, or community-based services
- Strong knowledge of disease processes, human behavior, and principles of growth and development
- Ability to provide effective therapeutic counseling and crisis intervention
- Excellent interpersonal and communication skills
- ability to collaborate in a team-based setting
Desired Qualifications
- CPI (Non-Violent Crisis Intervention) certification preferred
