Job Summary
Conduct evidence-based behavioral health screenings, assessments, and gender affirmation surgery letters within the primary care setting. Create and implement integrated treatment plans while providing crisis assessments and facilitating psychiatric hospitalization when necessary. Deliver consultations to medical providers and staff to ensure a coordinated continuum of care, maintaining accessible progress notes and organizing educational referral materials. Document all services per policy and state requirements, utilizing the Electronic Health Record to support integrated healthcare. Requires a Master's degree in Social Work, LCPC or LCSW-C license in Maryland, and clinical experience with the LGBTQ community.
Required Qualifications
- Minimum Master's Degree in Social Work
- LCPC or LCSW-C, in the state of Maryland
- Experience in the provision of outpatient mental health services
- Experience in a LGBTQ behavioral care
- Must have the ability to provide services that are relevant to the culture of the patients
- Must be able to identify and address disparities in healthcare access and quality
- Must be able to adapt services to cultural norms and promote inclusion among peers
- Must have clinical experience with the LGBTQ patients and community
- Must be familiar with open access model to care in community mental health and health care settings
- Must be able to adapt to changes in the field of Health Care
Desired Qualifications
- Additional knowledge of LGBTQ 50 plus community and/or children and adolescents
