Job Summary
Conduct brief, evidence-based screenings and time-limited assessments for patients with co-occurring behavioral health and substance use conditions. Create and implement integrated treatment plans while independently evaluating progress and modifying interventions as needed. Provide emergency crisis assessments and facilitate psychiatric hospitalization when required, then assess discharge readiness. Deliver therapeutic support to adults, children, adolescents, couples, and families through outreach and follow-up for non-engaged patients. Coordinate care by consulting with medical providers and staff, ensuring progress notes are accessible to the care team. Document all services per CBHC policy and state requirements, utilize the Electronic Health Record, and complete mandatory continuing education to maintain licensure.
Required Qualifications
- Master's Degree in Social Work, Counseling, Psy D or PhD", "Licensure in the state of Maryland
