Job Summary
Provide diagnostic assessment, onsite crisis assessment and intervention, and determine need for ongoing care based on clinical ability, medical, and psychosocial complexity. Develop treatment plans, implement specific interventions, and continually assess patient progress to revise diagnoses and plans accordingly. Conduct scheduled face-to-face therapy sessions to assess and improve emotional, mental, and behavioral health in a clinic setting. Document all treatment sessions and provide progress updates to the interdisciplinary team. May educate patient support members and refer for additional clinical services or to specialists as needed. Works within a small community and close-knit team with total independence and responsibility for patient well-being.
Required Qualifications
- Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences
- License in good standing in state(s) of practice
- Licensed Professional Counselor - Mental Health (LPC-MH)
- Licensed Marriage and Family Therapist (LMFT)
- Certified Social Worker - Private or Independent Practice (CSW-PIP)
- Credentialing through the Allied Health Staff of the organization
