Job Summary
Conduct single session therapy or ongoing therapy interventions in 20-40 minute sessions for patients with mental health, substance use, and physical health conditions. Provide same-day verbal feedback to primary care providers and educate them on brief behavioral and cognitive interventions. Participate in regular consultations with psychiatry, pharmacists, and community health workers to deliver integrated care, while performing risk assessments and providing psychoeducation to families. Write clear progress notes in the EHR and assist with program evaluation and fidelity measures. Utilize practice-based learning through literature review and outcomes data to improve interventions. Report to the Site Operations Director and engage in quality improvement initiatives supporting the Patient-Centered Medical Home.
Required Qualifications
- Master's degree in Social Work, Psychology, Counseling, or Marriage and Family Therapy
- LCSW, LMHC, or LMFT
- 1-2 years' experience of post-degree clinical experience preferred.
- Additional primary-care experience preferred.
- Strong organizational and time management skills.
- Proven ability to work well in a team environment.
- Excellent written and verbal communication skills.
- Ability to follow HealthLinc policies and procedures.
- Operate a multi-line phone system and other office equipment including printers, fax machines, etc.
- Basic computer skills (Microsoft Office, EHR, online sources, etc.)
