Job Summary
Coordinate client care by reminding clients of upcoming appointments, confirming attendance, and addressing barriers like transportation or scheduling conflicts. Transport clients to appointments, visits, or the workplace as needed, while building relationships with community partners to enhance program outcomes. Implement curriculum and training programs to maintain client placement in the community, provide structured skill maintenance activities, and create a supportive learning environment for adaptive functioning. Notify the team of missed appointments and collaborate on next steps. Represent Seneca in a positive manner while adhering to HIPAA, 42 CFR Part 2 regulations, and established standards for documenting services.
Required Qualifications
- High School Diploma or GED
- 18 years of age or older
- Valid driver's license and vehicle
- Demonstrated knowledge of basic computer skills
Desired Qualifications
- Experience working with individuals diagnosed with behavioral health and/or substance use disorder
