Transport clients to appointments, visits, or the workplace as needed, while building relationships with community partners to enhance program outcomes. Implement curriculum and training programs to maintain client placement in the community, provide structured skill maintenance activities, and create a supportive learning environment for adaptive functioning. Notify the team of missed appointments and collaborate on next steps. Represent Seneca in a positive manner while adhering to HIPAA, 42 CFR Part 2 regulations, and established standards for documenting services.
