Job Summary
Conduct behavioral health assessments to determine care needs, preferences, and goals for participants, then deliver and document interventions including symptom management, counseling, crisis intervention, and care transitions. Facilitate individual and group therapies such as art, music, and psychosocial drama to restore social roles, while actively participating in interdisciplinary team meetings to develop discipline-specific goals. Provide training on behavioral health symptoms and interventions to WelbeHealth staff. This role serves on a clinical interdisciplinary team following the PACE care model to advocate for the most vulnerable senior population. Master's degree in Behavioral Health or Doctorate in Psychology required with CA licensure.
Required Qualifications
- Master's Degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD)
- Licensed by CA Board of Behavioral Sciences as an LCSW
- Licensed by the CA Board of Psychologists as a Psychologist
- Minimum of five (5) years of behavioral health experience conducting therapy, completing behavioral health assessments, and managing care planning needs
- Minimum of two (2) years of experience with a frail or elderly population
- Proof of vaccination, or a vaccine declination form
