Job Summary
The Behavioral Health Specialist is a member of the Interdisciplinary Team (IDT) responsible for performing assessment and treatment activities for PACE Southeast Michigan participants presenting with behavioral health issues and diagnoses. Responsibilities include comprehensive biopsychosocial assessment, urgent consultations and crisis intervention, individual and group psychotherapy, substance use disorder treatment, and psychiatric hospitalizations. Responsibilities also encompass following up on recommendations and treatment plans, education and counseling on health-related biopsychosocial factors, accurate electronic health record documentation, ensuring quality and safety of services, collaboration with Primary Care Providers and Psychiatry, participation in treatment planning with the IDT, liaison roles during hospitalizations and SUD treatment, staff development on behavioral health topics, and contributing to policy improvement, committees, research, outreach, and training as assigned. Knowledge and abilities required include MSW with state licensure (MI), PCBH model expertise, experience with mental illness and co-occurring disorders, DSM-5 proficiency, patient-centered care, regulatory compliance, cultural competence, EHR proficiency, and strong collaboration skills. Required Qualifications
- Master’s Social Worker from an accredited school of social work
- Licensed to practice in the State of Michigan
- Limited License may be considered depending on experience and number of supervised hours
- Demonstrated knowledge of and experience with integrated Primary Care Behavioral Health (PCBH) model
- Extensive experience with mental illness, serious mental illness, co-occurring substance use disorders, and primary health care problems
- Expertise in DSM-5 criteria and psychology practices, treatments, and techniques with older adults
- Knowledge of and compliance with applicable local, state, and federal laws and regulations
