Job Summary
Conduct mental health assessments and develop personalized treatment plans for patients facing mental health challenges, addiction, and behavioral crises. Offer therapy tailored to adults and children with diverse needs while delivering crisis intervention and connecting clients to community resources. Collaborate with interdisciplinary teams to ensure holistic care and maintain accurate records to monitor patient progress. This role requires an active LPC or LMSW licensure and a Master's degree in Social Work or Counseling, with at least three years of experience working with the served population.
