Job Summary
Provide case management services to children and families with mental health and substance use diagnoses via telehealth, outpatient offices, and community-based settings. Monitor client symptoms and treatment response to coordinate immediate interventions, while developing service relationships with clients, families, and caregivers. Facilitate appointments for physical well-being, review diagnostic evaluations to implement individualized treatment plans, and provide crisis intervention consultation during emergencies. Coordinate community resources including schools and providers to ensure continuity of care and act as an advocate for clients. Maintain confidentiality of all documentation, attend clinical meetings and mandatory trainings, and perform administrative responsibilities regarding records and vendor compliance. Meet weekly with the Clinical Supervisor/Manager and maintain frequent contact with assigned professionals and outside agencies.
Required Qualifications
- High school diploma or equivalent
- Three years relevant experience
- Understanding of human behavior, mental illness, psychiatric symptoms, and impact on functioning and behavior
- Understanding how to therapeutically engage a mentally ill person
- Concepts of recovery/resiliency
- Understanding of the mental health system
- De-escalation techniques
- Understanding of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of emotional and mental dysfunctions
- Understanding of group behavior and dynamics
- Understanding of societal trends and influences
- Understanding of human migrations, ethnicity, and cultures and their history and origins
- Relevant experience or course of study in behavioral healthcare

- Flexible; able to work days, evenings, and/or weekends

- Ability to maintain relevant, timely documentation as required

- Must be able to access work locations, offices, and homes as necessary, which may include ascending/descending stairs
- Possess a valid Ohio Driver's License with a safe driving record and valid insurance
- Successfully complete pre-employment screenings, including drug tests, background checks, and any other required assessments, such as a physical examination and TB test
