Behavioral Health Specialist - LPC/LMSW

Behavioral Health Specialist at EPIC Health (Full Time, Hamtramck, MI) provides comprehensive outpatient mental health services within a MindPATH integrated model. Responsibilities include conducting mental health assessments, delivering outpatient therapy to adults and/or children, crisis intervention and stabilization, and maintaining a structured caseload of one patient per hour. Collaborates with Primary Care Providers, care managers, and interdisciplinary teams; handles documentation, progress notes, discharge planning, and case summaries. Requires a Master’s degree in Social Work or related field and an active LPC or LMSW license, with 3+ years of outpatient experience preferred. The role is hybrid (in-clinic and telehealth) with a focus on integrated, team-based care in an outpatient setting.
