Behavioral Health Specialist - LPC/LMSW

Job Summary
Provide comprehensive outpatient behavioral health care to adults and/or children in a hybrid in-clinic and telehealth setting. Conduct mental health assessments, develop individualized treatment plans, deliver outpatient therapy, and perform crisis intervention as needed. Maintain a structured caseload of one patient per hour, collaborate with Primary Care Providers within the MindPATH model and with care managers/interdisciplinary teams, connect patients with community resources, and document treatment plans, progress notes, and discharge planning. Thrive in a team-based, integrated care environment with strong clinical collaboration and a commitment to vulnerable populations. Background check required.
Required Qualifications
- Master’s Degree in Social Work, Counseling, or related field
- Active LPC or LMSW license
- Minimum 3 years of experience providing outpatient behavioral health services (preferred)
- Experience with assessments, treatment planning, therapy, and crisis intervention
