Job Summary
Conduct therapeutic interventions for participants in day centers and residences, including individual and group psychotherapy, crisis intervention, and psychiatric referrals. Independently assess behavioral health needs, coordinate care, and develop individualized treatment plans involving consultation with Primary Care Providers. Actively participate in the Interdisciplinary Team by attending staff meetings, contributing to care planning, and evaluating participant outcomes against established goals. Provide support to participants and families while advising staff on social, environmental, and cultural factors affecting health, and ensure accurate documentation in electronic medical records. Requires a Master's degree, one year of experience with the frail and elderly, and valid driver's license for home visits.
Required Qualifications
- Master's Degree from an accredited School of Social Work or equivalent (LPC or LLPC)
- Minimum of one (1) year working with the frail and elderly
- Valid Driver's License and transportation
- Ability to conduct home visits, required
- CPR certification or willingness to obtain
- Knowledge of human behavior principles, individual and group therapy techniques (preferably CBT and DBT), short-term interventions, substance abuse assessment and treatment, and relevant regulations
