Job Summary
Behavioral Health Specialist provides care coordination and clinical support for individuals with mental health needs within a multidisciplinary team. Responsibilities include conducting level of care assessments, addressing social determinants of health (housing, social supports, economic and education/vocational needs), developing and updating individualized treatment plans, ensuring seamless communication across external providers and internal teams, and assisting with transportation/appointments as needed. The role requires bilingual Spanish proficiency, a bachelor's degree in a related field, and Ohio driver’s license with reliable transportation; it involves documentation duties (clinical records, progress notes) and active engagement with community resources to support clients’ biopsychosocial needs.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology or a mental health related field
- Two years care coordination or case management experience
- Valid driver’s license in the State of Ohio with access to an insured vehicle for client transportation
- Bilingual in Spanish
- Excellent oral and written communication skills
- Ability to form effective therapeutic relationships and coordinate care plans
