Job Summary
Provide community psychiatric support and care coordination, including health promotion, comprehensive transitional care, and development of individualized treatment plans for clients. Collaborate with Nurses, Physicians, and Pharmacists to coordinate care, assist with transportation to appointments, monitor client status, and ensure follow-up. Maintain clinical records and timely progress notes; engage clients, families, and external providers to link to community resources and sustain biopsychosocial well-being. Must be bilingual in Spanish and hold a Bachelor’s degree in a related field, with a valid Ohio driver’s license.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology or a mental health related field
- Two years care coordination or case management experience
- A valid driver’s license in the State of Ohio with access to an insured vehicle
