Job Summary
In-person Behavioral Health Professional in Colorado counties provides brief intervention, assessment and treatment planning, crisis intervention, and care coordination for patients with chronic pain, depression, and anxiety. Responsibilities include real-time brief interventions, crisis response, documenting encounters, supervising feedback incorporation, conducting follow-up for high-risk patients, and serving as a team health educator. The role involves collaborating with care teams on interventions for patients with chronic illnesses and mental health diagnoses, assisting with staff training, mandatory reporting procedures, and contributing to a Quality Improvement program. Requires a relevant Master’s degree and licensure (LCSW, LPC, PhD, PsyD) and bilingual English/Spanish proficiency.
Required Qualifications
- Relevant Master’s Degree required
- Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), PhD, PsyD, required
- Experience with short term, strategic, goal-oriented therapy skills (SFBT) in a CHC or other health care setting
- Familiarity with current DSM and ICD Diagnosis Codes
- Sensitivity to low income, ethnic minority community
- Bilingual in English and Spanish, required
