Job Summary
Deliver primary care behavioral health services including consultation, psychotherapy, and care coordination for patients with acute, chronic, and severe mental health conditions. Conduct assessments using DSM and PHQ-9 tools to develop treatment plans, while utilizing evidence-based interventions such as Motivational Interviewing, CBT, and trauma-informed approaches. Provide on-site crisis triage and facilitate warm hand-offs to medical providers and specialized behavioral health services. Maintain accurate electronic health records in compliance with HIPAA and Joint Commission standards, and offer clinical supervision to non-independently licensed providers toward independent licensure. Collaborate closely with multidisciplinary teams to ensure seamless patient flow and integrated care delivery.
Required Qualifications
- Master's degree in a behavioral health field (i.e., social work, counseling, psychology)
- Three (3) years' post-graduate clinical experience in behavioral health, community health centers, primary care, or behavioral health settings
- Arizona Licensed Behavioral Health Professional; current, valid and in good standing
- Current certification in Basic Life Support (BLS) for health care providers
- Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment
