Job Summary
Provides rehabilitative mental health services to clients with mental illness or emotional disturbance in home, community, school, or office settings in alignment with each client’s treatment plan. May work with individuals, couples, families, or groups, and participate in care coordination, interdisciplinary team meetings, and documentation. Responsibilities include completing intake paperwork, screenings, functional assessments, releases, treatment plans, and ongoing outcome measurements; delivering Targeted Case Management; ensuring billable hours targets are met; and maintaining client confidentiality per HIPAA. Requires coursework in Behavioral Sciences, substantial supervised experience, background check, driving eligibility, and a clean period of substance use history. The role emphasizes integrated health across physical, mental, substance use, and oral care, with community activities and collaboration with community resources.
Required Qualifications
- Relevant coursework in Behavioral Sciences or related field (30 semester hours or 45 quarter hours)
- At least 2,000 hours of supervised experience delivering services to adults or children with mental illness or substance use disorder
- 4,000 hours of experience delivering services
- Must pass a criminal background check through MN DHS NetStudy
- Access to a personal vehicle and a valid driver’s license
- Insurable under agency automobile policy
- Free from substance use problems for at least 2 years
