Provide direct patient care, including assessments and care plan development, while managing a caseload and coordinating with the clinical team. Administer medications per physician orders, monitor patient progress, and adjust care plans as needed. Educate patients and caregivers on self-management, communicate with physicians to ensure continuity, and supervise home health aides and LPNs. Complete accurate OASIS documentation and visit notes while ensuring compliance with policies, regulations, and HIPAA standards. Requires an active RN license, BSN preferred, and home health experience. Salary range $74,000–$80,000 with Monday through Friday availability.
