Job Summary
Provide direct patient care, including assessments, care plan development, medication administration, and treatment per physician orders. Manage caseloads, coordinate with clinical teams, and monitor patient progress to adjust care plans as needed. Educate patients and caregivers on self-management while communicating with physicians to ensure continuity. Supervise and support home health aides and licensed practical nurses. Complete accurate documentation for OASIS, visit notes, and orders while ensuring compliance with policies, regulations, and HIPAA standards. Schedule is Monday through Friday with no weekend mandates.
Required Qualifications
- Active RN license
- Graduate of an accredited nursing program
- Home health or related behavioral health clinical experience
- Strong communication, organization, and critical thinking skills
- Current CPR certification
- Valid driver's license and reliable transportation
