Job Summary
Provide direct patient care, including assessments and care plan development. Manage a caseload and coordinate care across the clinical team while administering medications and treatments per physician orders. Monitor patient progress, adjust care plans as needed, and educate patients and caregivers on self-management. Communicate with physicians and care teams to ensure continuity of care and supervise support staff like HHAs and LPNs. Complete accurate documentation for OASIS and visit notes while ensuring compliance with policies, regulations, and HIPAA standards. Requires an active RN license, BSN preferred, and home health or behavioral health experience. No weekend mandates available with Monday through Friday schedules.
Required Qualifications
- Active RN license
- Graduate of an accredited nursing program
- Home health or related behavioral health clinical experience
- Strong communication, organization, and critical thinking skills
- Current CPR certification
- Valid driver's license and reliable transportation
