Behavioral Health Nurse- Mornings

Provide direct patient care in a home health behavioral health setting, including assessments and care plan development; manage a caseload, coordinate care across the clinical team, administer medications and treatments per physician orders, monitor patient progress, educate patients and caregivers on care and self-management, communicate with physicians and care teams to ensure continuity of care, supervise and support HHAs and LPNs, complete accurate documentation (OASIS, visit notes, orders), and ensure compliance with policies, regulations, and HIPAA standards.
