Job Summary
Registered Nurse Care Manager for Behavioral Health at Ellington Place in Kansas City, MO. The RN will act as a clinical coordinator and health advocate, bridging psychiatric care with general medicine for adults with severe and persistent mental illness residing in residential programs. Responsibilities include holistic care coordination across psychiatric and chronic medical needs, interdisciplinary collaboration with psychiatrists, primary care physicians, specialists and pharmacies, medication reconciliation during transitions, MAR management across the residential program, and providing medical guidance to residential staff on health monitoring, potential medication side effects, and chronic disease management. Required Qualifications
- Current Missouri license as a Registered Nurse (RN)
- BSN or ADN with relevant experience
- Minimum of three (3) years experience in a healthcare setting
- Excellent written and verbal communication skills
- Must be able to read and speak English
- Proficient in Word, Excel and PowerPoint
- Current BCLS
