Job Summary
Nurse (Behavioral Health) – provide direct patient care and care coordination in a home-health/behavioral health setting. Responsibilities include assessments, care plan development, medication administration per physician orders, monitoring patient progress, patient and caregiver education on self-management, collaboration with physicians and care teams, supervising HHAs and LPNs, and completing timely documentation (OASIS, visit notes, orders) while ensuring HIPAA compliance. Requires active RN license, BSN preferred, and experience in home health or behavioral health clinical settings; CPR certification and valid driver’s license with reliable transportation are needed. Role emphasizes autonomy, patient-centered care, and compliance within a collaborative team.
Required Qualifications
