Job Summary
Provide direct patient care, including assessments and care plan development, while managing a caseload and coordinating across the clinical team. Administer medications and treatments per physician orders, monitor patient progress to adjust plans, and educate patients and caregivers on self-management. Communicate with physicians to ensure continuity, supervise and support HHAs and LPNs, and complete accurate documentation for OASIS and visit notes. Ensure compliance with policies, regulations, and HIPAA standards. Requires an active RN license, BSN preferred, home health experience, and current CPR certification. No weekend mandates; Monday through Friday schedules available.
Required Qualifications
- Active RN license
- Graduate of an accredited nursing program
- Home health or related behavioral health clinical experience
- Current CPR certification
- Valid driver's license and reliable transportation
