Behavioral Health Nurse- Afternoon/Evenings

Provide direct patient care, including assessments, care plan development, and medication administration per physician orders. Manage caseloads, coordinate with the clinical team, and educate patients and caregivers on self-management. Monitor patient progress to adjust care plans and ensure continuity through communication with physicians. Supervise and support HHAs and LPNs while completing accurate OASIS documentation and visit notes. Maintain compliance with policies, regulations, and HIPAA standards. Requires an active RN license, BSN preferred, and home health experience. No weekend mandates available.
