Behavioral Health Liaison: Integrated Care for Kids (InCK)

Job Summary
Coordinate referrals and track status; facilitate information sharing between CBH clinical teams and system partners; meet with members to provide encouragement and support for behavioral health services; conduct community outreach and psychoeducation; help members identify and address barriers to treatment; follow up with members and providers by community visits and phone; maintain accurate daily logs and CBH documentation; participate in supervision and assigned meetings; cover for peers and contribute to special projects; on-site at St. Christopher’s Hospital for Children and support integration of care with the Integrative Care team. Requires Bachelor’s in Social Work or related field, PA Child Abuse Clearance, minimum 2 years direct service experience, HIPAA compliance, strong communication and organizational skills; bilingual Spanish a plus.
Required Qualifications
- Bachelor’s degree in Social Work or related field
- PA Child Abuse Clearance required
- Minimum of 2 years of direct service experience including assessment, counseling and coordinating services
- Familiar with services for adults and children in the managed care system
- Excellent interpersonal and collaboration skills
- Excellent verbal and written communication skills
- Bilingual, Spanish speaking skills a plus but not required
- Proficiency with Microsoft Office and ability to type 40 words per minute
- Strong organizational skills with the ability to multi-task in a high-volume environment
- Ability to work independently and as part of a team
- HIPAA compliance
- Philadelphia residency/relishment within six months of hire
