Behavioral Health Integration Clinician - Oregon

Job Summary

Screen and assess patients for mental health and substance abuse disorders, then develop evidence-based treatment plans using techniques like motivational interviewing and behavioral activation. Provide psychotherapy, support psychotropic medication management, and deliver health behavior interventions to improve medical outcomes. Systematically track treatment response and monitor patients for clinical changes via in-person, video, or telephone encounters, documenting all progress in the EHR. Facilitate referrals to psychosocial treatments and coordinate care with medical providers and psychiatric consultants through weekly caseload consultations. Actively participate in process improvement by identifying workflow inefficiencies and meet monthly performance metrics. Work primarily remotely with three days of in-clinic attendance, utilizing high-speed internet and multiple software applications to manage the assigned caseload.

Required Qualifications

  • A minimum of a master's degree in social work, Psychology, Counseling, Rehabilitation, or other relevant field
  • 3-5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination
  • Working with various populations, including older adult Medicare populations, who have chronic medical and behavioral health challenges
  • Active independent clinical license in Oregon
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Professional Counselor (LPC)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Psychologist
  • Current enrollment in the state's Medicaid & Medicare is preferred but eligibility for such at a minimum is required
  • License eligible candidates who have completed their board exam and are completing clinical hours under supervision are eligible to apply
  • The position is primarily remote with the majority of services provided telephonically or via video; however, your work location in the designated state is required
  • Given the remote nature of the position, stable high-speed internet is expected
  • Proficiency in operating computer systems and utilizing multiple software applications is required
  • The ideal candidate would be proficient in utilizing electronic medical records, scheduling systems, remote phone systems and video systems
  • Schedule: Monday-Friday, 8:00 AM to 4:30 PM
  • Hybrid: Minimum of three days a week in the clinic

Desired Qualifications

  • Current enrollment in the state's Medicaid & Medicare is preferred
