Job Summary
Screen and assess patients for mental health and substance abuse disorders, then facilitate engagement, treatment plans, and follow-up care. Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, motivational interviewing, and problem-solving treatment. Support psychotropic medication management by monitoring adherence, side effects, and treatment effectiveness while tracking clinical symptoms and outcomes via EHR. Deliver health behavior and social determinants interventions, provide patient education on disorders and treatments, and facilitate referrals to evidence-based psychosocial services. Participate in weekly caseload consultations with the psychiatric consultant to communicate recommendations to medical providers and adjust treatment plans as needed. Document all in-person, video, and telephone encounters in the EHR, track follow-ups, and actively contribute to process improvement initiatives. Work Monday through Friday, 8:00 AM to 5:00 PM Eastern time in a primarily remote environment with periodic in-person clinic visits.
Required Qualifications
- A minimum of a master's degree in social work, Psychology, Counseling, Rehabilitation, or other relevant field
- 3-5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination
- Working with various populations, including older adult Medicare populations, who have chronic medical and behavioral health challenges
- Active independent clinical license in Michigan
- Licensed Master Social Worker - Clinical (LMSW)
- Licensed Professional Counselor (LPC)
- Licensed Psychologist
- Current enrollment in the state's Medicaid & Medicare is preferred but eligibility for such at a minimum is required
- License eligible candidates who have completed their board exam and are completing clinical hours under supervision are eligible to apply
- The position is primarily remote with the majority of services provided telephonically or via video

- Proficiency in operating computer systems and utilizing multiple software applications is required
- The ideal candidate would be proficient in utilizing electronic medical records, scheduling systems, remote phone systems and video systems
