Behavioral Health - Integrated Care Advocate

Job Summary
Support and coordinate patients' mental and physical health care by assisting with severe behavioral health concerns, triaging emergencies, and managing Medication Assisted Treatment programs. Collaborate with primary care teams to promote integrated care, provide behavioral interventions, and educate patients on chronic condition prevention. Monitor treatment plan goals, connect patients to community resources, and respond to organizational emergency codes. Maintain timely electronic health records and perform administrative support for the Integrated Care team while delivering services in person or via telehealth.
Required Qualifications
- Bachelor's degree in social work, human services, public health, or similar
- Current Basic Life Support (BLS)
- Ability to speak clearly and effectively to demonstrate knowledge and convey health education principles to a variety of audiences
- Ability to hear average conversations and respond accordingly
- Ability to read and comprehend communication and instructions from a variety of sources
- Ability to effectively communicate in writing in the form of e-mails and required reports
- Ability to handle emotionally charged situations and maintain professionalism
- Ability to occasionally lift and/or move up to 30 pounds
Desired Qualifications
- Minimum of two years' experience in behavioral health services
- Experience in behavioral health, integrated care or an FQHC health center
- Bilingual abilities
