Behavioral Health Counselor

Job Summary
Provide behavioral health assessments, develop individualized treatment plans with measurable goals and evidence-based interventions, and deliver individual, group, and family therapy for mental health and substance use concerns. Perform crisis intervention and safety assessments, coordinate care with medical providers and care teams, complete referrals to community resources, and maintain accurate, confidential documentation in the EHR. Collaborate in multidisciplinary teams within a Federally Qualified Health Center, promoting trauma-informed, culturally responsive, patient-centered care. Requires active Louisiana licensure (LCSW or LPC) and a Master’s degree in Social Work or Counseling; travel between sites may be required. Includes required clinical skills in assessments, diagnoses, treatment planning, and evidence-based practices; strong communication and crisis intervention abilities; familiarity with HIPAA and integrated care.
Required Qualifications
- Active Louisiana licensure as an LCSW or LPC
- Master’s degree in Social Work or Counseling
- Experience conducting assessments, formulating diagnoses, and developing treatment plans
- Knowledge of evidence-based practices including trauma-informed care and co-occurring disorders
- Working knowledge of DSM and ICD diagnostic frameworks
- Proficiency with electronic health records and timely clinical documentation
- HIPAA/confidentiality requirements and integrated care best practices
- American Heart Association BLS for Healthcare Providers required
