Job Summary
Facilitate patient involvement in treatment and education programs while performing individual, group, and family counseling sessions. Conduct utilization review and discharge planning to ensure safe patient transitions, and assess psychosocial risk factors through evaluation of functioning, support systems, and coping skills. Counsel patients and families regarding emotional, social, and financial impacts of illness, then access and mobilize community resources to meet identified needs. Serve as the point person for child, adult, and elderly abuse/neglect cases, collaborating with protective teams and providing intervention in guardianship, foster care, and adoption placements. Implement evidence-based practices such as CBT, MI, and DBT, maintain accurate documentation, and develop strong relationships with community agencies. Requires a Master's degree, Indiana LCSW/LMHC/LMFT license, and 0-3 years of experience. Full-time day shift at White Memorial Hospital.
Required Qualifications
- Master's Degree from an accredited college or university in Social Work, Counseling, Psychology, or a related field
- Requires 0-3 years of relevant experience
- License eligible in Indiana as an LCSW, LMHC, LMFT, or LCAC
- Must have a minimum of a permanent LSW, LMHC-A, or LMFT-A upon hire
- Certification in crisis intervention
- BLS certification
- Relevant healthcare, mental health, or community-based experience
- Knowledge of the impact of disease processes and human behavior to provide psychosocial assessments, therapeutic counseling, and crisis intervention
- Knowledge and application of evidenced-based practices such as CBT, MI, and DBT
- Knowledge of state and county legal mandates related to child and adult protective services
- Understanding of growth and development principles across the lifespan
- Ability to apply principles and techniques of crisis intervention and de-escalation
- Ability to create a safe, therapeutic environment, including instruction of daily activities
- Basic proficiency in Electronic Medical Record systems and MS Office applications (Word, Excel, PowerPoint)
- Ability to develop and maintain strong relationships with community agencies and other stakeholders
