Behavioral Health Counselor

Deliver exceptional patient care through psychotherapy services to individuals, families, and groups via in-person and telehealth settings. Conduct ongoing assessments of mental health, substance use, and psychosocial needs while developing, implementing, and updating individualized treatment plans. Collaborate with psychiatric nurse practitioners, primary care providers, and case managers to ensure comprehensive care, and partner with external agencies to assess high-risk patients. Ensure proper documentation in the AHC electronic health record and work with data analysts to monitor key performance indicators and improve patient outcomes. Maintain licensure and CEU requirements, participate in continuous quality improvement projects, and collaborate with the revenue cycle team for financial accountability.
