Job Summary
Conduct assessments, consultations, and targeted, behaviorally based interventions with patients and families within a primary care setting. Evaluate crisis situations and apply appropriate interventions while assisting clients in achieving goals related to mental health and lifestyle factors. Partner with primary care, dental, social services, and addiction treatment professionals to develop behavioral interventions and facilitate referrals to specialty providers. Prepare and maintain all required documentation through a shared electronic health record and assist with tracking behavioral health patient registries. This role reports to the Integrated Care Director and requires the ability to work in both Larimore and Grand Forks locations. Candidates must hold an LCSW, LMSW, or LPCC license and demonstrate cultural humility.
Required Qualifications
- Master's degree in counseling, social work, psychology, or related behavioral health sciences
- Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) or Licensed Professional Clinical Counselor (LPCC) or Licensed Professional Counselor (LPC) licensure
- Working knowledge of motivational interviewing and other evidence-based interventions
- Comfort working within a primary care setting and with an interdisciplinary team
- A belief in the principles of trauma informed care and harm reduction
- Strong written and verbal communication skills
- Ability to work in both the Larimore and Grand Forks locations
Desired Qualifications
- Experience in a Primary Care Behavioral Health Model
- Prior experience working with diverse population groups
- Experience and/or knowledge of evidence-based treatment and intervention for people experiencing substance use disorders
