Job Summary
Delivers brief consultation-based services to community and primary care patients, including client screening, assessment, and interventions within the integrated behavioral health model. Facilitates classes, groups, and social service referrals while developing measurable, time-limited treatment plans. Collaborates with community resources to coordinate comprehensive multi-disciplinary care and serves as a resource for patients, families, and healthcare employees. Maintains clinical documentation, participates in audit reviews, and attends required trainings and meetings. Requires a Master's degree, active Arizona behavioral health license, and 1+ years of consultative experience.
Required Qualifications
- Knowledge of behavioral health services and standards
- Excellent verbal and written communication skills and ability to interact with individuals of various social, cultural, and educational backgrounds
- Ability to understand and follow written and verbal instructions
- Excellent analytical and organizational skills
- Excellent problem-solving and critical thinking skills
- Detail-oriented and professional
- Ability to exercise confidentiality, discretion, and good judgment
- Proficient with Microsoft Office Suite or related software
- Ability to speak English
- Master's degree in Social Work, Counseling, or other behavioral health related field
- Actively licensed as: LCSW (Licensed Clinical Social Worker) or LPC (Licensed Professional Counselor) or LMFT (Licensed Marriage Family Therapist) or Licensed Psychologist
- License must be in good standing with Arizona Board of Behavioral Health Examiners
- 1+ years of providing behavioral health consultative services or other related experience
- 3+ years of healthcare experience
- New Hires are required to pass pre-employment background check and drug testing
- Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire
- Basic Life Support (BLS) certification
