Job Summary
Provide behavioral health services within an integrated primary care setting, including consultation and co-management of mental and psycho-social issues; deliver brief, evidence-based behavioral interventions and develop psychotherapeutic/psycho-educational treatment plans; assess and diagnose using DSM or ICD-9 frameworks for referred patients; collaborate with an interdisciplinary team to promote self-management and improve health outcomes; responsibilities include crisis triage on-site, coordination with primary care, and developing protocols for managing chronic conditions; required to hold a Master’s degree in Behavioral Health with two years of related experience and to have certification or eligibility as an LPC, LCSW, or Psy.D.; bilingual English/Spanish preferred; must comply with vaccine mandates and maintain CPR/AED certification; located at Adelante Healthcare sites in Surprise, Mesa, or Goodyear, AZ.
Required Qualifications
- Master's degree in the Behavioral Health field (Psychology, Social Work) with two (2) years of directly related experience is desired
- Certification or certification eligibility as an LPC, LCSW, or Psy.D. is required
- Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions
- Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions
- Bilingual (English/Spanish) is preferred
- CPR and AED certification through AHA/Red Cross guidelines
