Behavioral Health Consultant (LCSW, Psychologist, LPCC) Mead Valley

The Behavioral Health Consultant will provide direct patient care for behavioral health services within an integrated care model, delivering brief targeted psychotherapy and conducting initial/ongoing assessments to develop patient care plans. Responsibilities include evaluating social, emotional, and economic factors affecting care, providing brief psychotherapy to patients and families, incorporating co-occurring disorders into plans, crisis management and psychoeducation, warm hand-offs at provider requests, and collaborating with primary care teams to support medical treatment and quality assurance. The role requires CA licensure/licensure track (CA Psychology, LCSW, or LPCC as appropriate) and a minimum of two years of pre/post licensure psychotherapy experience, with bilingual English/Spanish preferred.
