Behavioral Health Consultant - Fox Chapel - Full Time

Conduct clinical mental health consultation services for patients referred by the care team, providing immediate assessment, intervention, and referrals for mental health and addiction needs. Deliver 15-30 minute therapeutic counseling sessions using solution-focused approaches, CBT, and motivational interviewing to address functional outcomes and facilitate skill building. Engage patients via telephone and in-person visits to promote ongoing change efforts, re-engage non-compliant enrollees, and connect them to community resources for social determinants of health. Participate in daily huddles, practice staff meetings, and population health efforts to develop clinical pathways and provide education on behavioral health topics. Support the care team in managing psychosocial aspects of chronic and acute diseases through brief follow-up and relapse prevention education.
