Behavioral Health Consultant

Works as a member of the healthcare team to assist primary care providers in managing patient health by providing brief consultation, assessment, intervention and education for behavioral health issues. Performs targeted strengths-based assessment and evaluation, formulates and implements brief behavioral health interventions using modalities such as Motivational Interviewing, Cognitive Behavioral Therapy, Mindfulness, Solution Focused, and Acceptance and Commitment Therapy. Facilitates smoking cessation with evidence-based treatments, communicates findings to providers, documents concisely in the EMR, triages to specialty care when needed, and collaborates with external agencies. Travel between offices as required and maintains professional relationships to promote psychological and physical health.
